Returning customers: Please note that with our new website, our customization order form has changed. You can now enter your jerseys one-at-a-time for each item that you are sending in your package. The order form is now a two-part process: First add each item, then confirm your contact and shipping information.

Exclusive Pro Sports does not sell professional sports (MLB, NHL, NFL, etc.) jerseys. This order form is only for customization.

How to fill out the customization form:

Each entry into this form should contain a description and instructions for one jersey. If you have multiple identical jerseys that are to be customized in the exact same way, you may note that with the quantity field.

Please be sure to fill in the Quantity, Jersey Name/Description and Customization Instructions fields. If you are requesting customized names and/or numbers, please be sure to fill those fields in too.

Please be specific when entering customization instructions.

After entering your first jersey, you may keep using this form to enter all of the jerseys you plan on sending in the same package.

When you submit this form, you will be taken to a screen that will show you a listing of all jerseys you have entered in this current order. Think of it just like the shopping cart of an online store. You will be able to use this form to enter each jersey you are sending in a single package.

You will have the opportunity to edit, change or remove items prior to completing your order.

Return shipping is not free. Please reference the shipping quotes information at the bottom of this page for the current rates.

Be sure to print and include any reference photos, specific directions, measurements, or anything else you want EPS to know when fulfilling the order, color prints are preferred.

You will be given an order number upon completion of your order. Please be sure to include that order number with the jersey(s) you are sending us.

You will receive an order confirmation via email when you complete your order. You can print that confirmation and include it in your package if you wish.

Please check your SPAM folder if you do not receive an order confirmation.

Each jersey is subject to a $50 cancellation fee.

If you want to verify your package has been received, please use a tracking number when shipping, we DO NOT contact customers regarding safe arrival. There are no changes, add-ons or combining orders once an order has been placed.

Shipping Quotes:

EPS recommends insurance for all packages. We can use any shipping carrier preferred, UPS is our default, which includes a tracking number and insurance. We can also use a customer's shipping number, please include the number on the Order Form. If cheaper shipping is desired or another method preferred, please state on the Order Form. Shipping quotes are subject to change based on location. International shipping and HI, AK, will be assessed on completion. All quotes include processing and handling. Jerseys received in multiple packages or at different times cannot be combined by EPS for return shipping. They will be returned as they were received.

Due to rising costs from all shipping providers - our costs have had to increase as well. Shipping quotes are available upon request.
